V-Safe is a smartphone-based tool that uses text messaging and web surveys to provide personalized health check-ins after you receive a COVID-19 vaccination. Through v-safe, you can quickly tell CDC if you have any side effects after getting the COVID-19 vaccine. Depending on your answers, someone from CDC may call to check on you. And v-safe will remind you to get your second COVID-19 vaccine dose if you need one. Your participation in CDC’s v-safe makes a difference — it helps keep COVID-19 vaccines safe.

What to bring to your Nambe Drugs/Los Alamos County vaccine appointment
- Print, complete and sign Nambe Drugs CONSENT FORM and bring it to your first and second appointments (pdf document)
- Your NMDOH Confirmation number (seven digit number issued upon registration with NMDOH).
- Insurance and Medicare cards if applicable
- On the morning of your appointment (first and second appointments), complete the NMDOH MEDICAL QUESTIONNAIRE on your NMDOH profile online.
- CDC Vaccine Card, if this is your second appointment. This card will be updated after your booster.
- To be eligible for the $100 NMDOH incentive - bring proof of residency in New Mexico (identification card, pay stub or utility bill); and if this is your booster don't for forget the CDC Vaccine Card (previous bullet). If you've misplaced your vaccine card visit http://vaxviewnm.org to obtain a copy of your vaccine records or call 833-882-6454.
