Here are a variety of forms here to help you complete utilities and county-wide transactions. Some  forms can be filled out online and others can be printed out, completed and then returned to the DPU, via mail (170 Central Park Square, Los Alamos, NM 87544), or in person, or by fax at 505-662-8005. In all cases, if a signature is required, please sign manually.  We are unable to accept electronic signatures at this time.  Thank you!
