Why Read This:  Forms to accomplish a variety of objectives with the DPU and the county can be found here. Those labeled "fillable online" can be completed using your computer; otherwise the form needs to be printed out and filled out by hand.
Also of Note:  The DPU and the 311 Customer Care Center are working together to automate the tracking and handling of general county inquiries and more complex utility issues through the "Your Account" page. If you can't find the form you need here, click on "Your Account" then use the "Contact Us" button on the left side, to request the form desired.

Here are a variety of forms here to help you complete utilities and county-wide transactions. Some  forms can be filled out online and others can be printed out, completed and then returned to the DPU, via mail (901 Trinity, 2nd Floor, Los Alamos, NM 87544), or in person, or by fax at 505-662-8005. In all cases, if a signature is required, please sign manually.  We are unable to accept electronic signatures at this time.  Thank you!
